First visit

Upon arrival to your first appointment you will be asked to present your insurance card and fill out our patient information forms.  These forms are available to be printed and filled out below. Your first appointment will focus on evaluation while you participate in a one-on-one meeting with one of our licensed physical therapists.  During the evaluation your physical therapist will obtain a complete medical history and conduct a thorough physical examination.  From this evaluation your physical therapist will develop a treatment plan to assist you in achieving a quick recovery.

Follow-up visits

Your follow-up appointments will last between 45 minutes and one hour and will encompass any treatment techniques, modalities, and exercise that your physical therapist believes will help you to accomplish your goals.

What to wear

You will need to wear comfortable clothing to all of your appointments including the initial evaluation.  This will allow your physical therapist to see and work with your injury and will also allow you to complete any stretches and exercises necessary to speed your recovery.

Insurance

We will bill your insurance company as a courtesy to you and are in-network with most insurance plans.  Please call if your company is not listed below to see whether or not your plan covers you for treatment at Ashland Physical Therapy.
